Community Mental Health Transformation Programme

Local health care providers, including GPs, mental health services, councils, and voluntary sector organisations, are working with people who have experienced mental health problems to help shape mental health services in Staffordshire and Stoke-on-Trent.

The aim of this transformation is to help those with severe mental illnesses to access the care and support they need close to where they live. This will help to achieve a more effective treatment and support plan no matter the diagnosis or severity of the illness.

Getting involved

An “Involvement Collaborative” has been set up by Support Staffordshire, Midlands Partnership NHS Foundation Trust (MPFT) and North Staffordshire Combined Healthcare NHS Trust (NSCHT) to create a consistent approach to make sure that people with lived experience are involved in decision-making and evaluation throughout the transformation programme.
