Children’s Community Nursing Team (CCNT) engagement survey | Midlands and Lancashire CSU

The six Staffordshire and Stoke-on-Trent Clinical Commissioning Groups (CCGs) are currently working in conjunction with the Staffordshire Community Children’s Nursing Team (CCNT) to engage with their patients.

Historically, the CCNT service specification for the three geographic localities has been different. In the 2019/20 contract, the service development implementation plan identified a need to realign the service equitably across Staffordshire. As a result of the proposed service redesign, there is a need to engage with children and their families who use the service and clinicians to explain the proposed changes which specifically includes changing the hours that business-as-usual telephone and visit support will be available across the three localities.

The aim of the engagement is to reassure patients and GPs that the proposed changes to telephone support and visits are minimal and aims to provide a more equitable service across Staffordshire, utilising all available resources to support service users more effectively.

At present, the CCNT is delivered through three locality teams (North, East, and West Staffordshire) who provide nursing support to facilitate early discharge or prevent admission to hospital for children with a range of short-term (acute) illnesses or health problems. In addition, CCNT provides support to patients with long-term health conditions and end-of-life care which includes planning care with family alongside other professionals.

In addition, the existing on-call telephone support, which is available for service users with a confirmed palliative diagnosis in the North locality, will become the responsibility of an on-call palliative care consultant at University Hospital of North Midlands NHS Trust (UHNM) via the Children’s Assessment Unit. This will ensure 24/7 support for these families when required, where care can be managed or escalated appropriately.

All children in the last few weeks of life will still have 24/7 access to the Children’s Community Nursing Team for symptom management and any other nursing, parental support required.
